Employment since 18th birthday

Residences for past five years.

Details of any prior visa applications.

For each parent, child and sibling:

Full name

Current address

Date of birth

Date of death and

marital status

passport number and expiry date

citizenship

Two (2) passport-size pictures of each applicant

Passport (identity & validity pages only)

Birth certificate

Identification card

Education documents (diplomas, degrees, certificates, etc.)

Proof of income, assets, pension, property deeds, liabilities

Employment references (letters from every employer you have worked for)

Police clearance certificate for each country of residence for last ten years.

Marriage certificate

Divorce Decree and / or Separation agreement and child custody order

Non-refundable Government processing fees
