• Current telephone number and email address

• Marital status

• Two photographs, 35 mm wide by 45 mm tall

• Passport, including identity and validity pages

• Driver’s licence

• Record of Landing document (IMM 1000)

• Recent tax assessment from the Canada Revenue Agency

• Birth certificate

• Education history

• Employment history for the past five years

• Residence history for the past five years

• List of absences for the past five years, by date (dd/mm/yyyy), including location and reason for absence.
